Evoke Support is a Northwest-based provider of Supported Living and Outreach services. We specialise in supporting adults and young people (16+) to live fulfilling and independent lives in their own homes and communities. We provide bespoke packages to meet the individual needs of those with learning disabilities, acquired brain injuries, mental health conditions, autism, complex needs, and physical disabilities. 

Our services are designed to help individuals live safely, confidently, and independently—whether at home or in the community. From daily living support to more complex care, we work closely with each person, their families, and professionals to create support plans that meet real needs and goals.

Evoke Support is built on firm foundations. Our leadership team brings together over 57 years of operational experience in social care, giving us the insight and expertise to offer truly person-centred, responsive support.

At the heart of our approach is a belief that everyone deserves the opportunity to lead a meaningful life with choice, dignity and respect. We work in partnership with individuals, families, carers, and professional bodies—delivering person-centred care plans that reflect the goals and preferences of those we support.

Whether through practical daily assistance or helping someone reconnect with their community, we are here to make a real difference—providing care that is not only compassionate and professional but flexible, inclusive, and empowering.

Self Directed Support / Direct Payments / Individual Service Funds (ISFs), 

As a flexible provider, we accept referrals / enquiries from or on behalf of individuals who are in receipt of Direct Payments, Individual Service Funds (ISFs) etc.
