Greater Manchester Transition Framework

This framework is a self-assessment tool designed to enhance person-centred transition planning for post-11 education providers in Greater Manchester. It focuses on key transitions between Key Stages 3 to 4 and Key Stages 4 to 5, aiming to foster inclusivity, equity, and individual growth for all learners.

The framework promotes holistic, high-quality transition programmes that support every learner's unique needs and aspirations, ensuring informed decision-making about future education and careers while promoting equity and inclusion. Schools and Colleges who can demonstrate adherence to this framework should ensure that this is clearly identified within their website to demonstrate their commitment to best practice.

This document is for use by:

· Senior Leaders: Ensuring strategic oversight and whole school/college implementation.

· Transition Leads: Coordinating processes and fostering collaboration between educational and workplace settings.

· Careers Leaders & Advisors: Embedding high-quality careers education and transition planning.

· SENCos: Ensuring learners with additional needs receive tailored support.
