NEW - REQUEST FOR EARLY HELP SUPPORT FORM
From 1st Jan 2021 a slightly revised Request for Early Help Support form will come into effect. This form should be completed by partner agencies requesting targeted Early Help interventions when those families are unable to be supported by universal or single agency services. To access the form click here.
The form should be completed with the agreement of the parent/carer and when age apropriate the young person. If there is a lead professional for the child or family an Early Help Assessment should have been completed prior to the request for support form being submitted.

HOW DO I REGISTER AN EARLY HELP ASSESSMENT?
Please register a completed Early Help Assessment by sending an email to:

If you have sent an Early Help Assessment (EHA) to the Early Help Team for registration and not received a response, or an Unique Reference Number (URN), returmed to you 10 days after sending, please contact the appropriate Early Help Hub to check if your EHA has been registered.
